Where Were the Errors Found?
The report from the Senate Finance Committee found errors throughout the medical transplant system, including:
- Doctors failing to identify diseases in donated kidneys, hearts, and livers
- Errors when attempting to match organ donors with recipients of the same blood type
- Delays in blood and urine tests to ensure donors and recipients are compatible prior to transplantation
- Healthy donor organs being accidentally disposed of
- Donor organs being lost during transport

As part of its investigation, the Senate Finance Committee looked at 1,118 complaints filed with the United Network for Organ Sharing (UNOS), which oversees most organ procurement organizations. After reviewing a recent UNOS report, the committee determined that 70 people died and 249 developed serious injuries due to screening errors during a recent eight-year window.
